MOTOR CLAIM

What to do & documents needed for motor insurance claim

General Documentary Requirements

  • Policy/Endorsement
  • Non-Waiver Agreement (Form to be provided by the Adjuster)
  • Sworn Statement of Claim (Form to be provided by the Adjuster)
  • Affidavit of Loss
  • Pictures

Own Damage (OD) / Acts Of Nature (AON) / Total Loss

ASSURED

  • OR/CR – Certificate of Registration and Official Receipt
  • Driver’s License; if not yet available, the latest OR Any of the following:
    • Police Report – if the claim has recovery, the original copy must be submitted
    • Affidavit of the driver or owner of the vehicle
    • FGen Motor Vehicle Accident Report Form
  • Pictures showing the plate no. and the extant of damages sustained (clear copy)

REPAIR SHOP

  • Repair Estimate
  • Picture or Stencil of Serial & Motor Numbers

CPTL Claim / ETPL – BODILY INJURED and/or DEATH Claim (ETPL-BI)

ASSURED
Documents may be sent via e-mail:

  • OR/CR – Certificate of Registration & Official Receipt
  • Driver’s License; if not yet available, the latest OR
  • Pictures showing the plate no. and the extant of damages sustained (clear copy)
  • Stencil of Serial & Motor Numbers

Must submit the Original Copy or Certified True Copy of the following:

  • Police Report
  • Medical Certificate
  • Hospital Bills and/or Medical Receipt

CPTL Claim / ETPL – BODILY INJURED and/or DEATH Claim (ETPL-BI)

CLAIMANT

For Death Claim:

  • Must submit the Original Copy or Certified True Copy of the following:
  • Death Certificate
  • Proof of Affiliation of the Claimant (Birth Certificate or Marriage Contract or Affidavit)

ETPL – Third Party-Property damage (ETPL-PD)

THIRD PARTY

  • OR/CR – Certificate of Registration & Official Receipt
  • Driver’s License; if not yet available, the latest OR
  • Certificate of No. Claim from existing insurance company
  • Pictures showing the plate no. and the extant of damages sustained (clear copy)
  • Repair Estimate
  • Picture or Stencil of Serial & Motor Numbers

CARNAP Claim

Documents may be sent via e-mail:

  • OR/CR – Certificate of Registration & Official Receipt
  • Driver’s License; if not yet available, the latest OR

Assured must submit the Original Copy the following:

  • Police Report on the theft of carnapping incident / Alarm Sheet
  • Copy of Complaint Sheet with the Headquarters Traffic Management Group, PNP Crame.
  • Certificate of Non-Recovery
